Apply For Food Stamps & Medicaid Benefits in Florida


The State of Florida offers food stamps and Medicaid to qualifying low-income individuals. Low-income individuals and families may qualify for food stamps. Medicaid has stricter guidelines, requiring individuals to meet income requirements and be elderly, disabled, a pregnant woman or a child.


Instructions


1. Utilize the pre-screening tool on Florida's Access Florida website. You will be asked questions regarding your residency, household size, total income and whether or not you have a disability and assets. If you are determined to be eligible, proceed to Step 2.


2. Complete and submit a Medicaid application online on Florida's Access Florida website. You will be asked questions regarding your sources of income and resources and will need full name, date of birth and Social Security number for each household member. Check food assistance and medical assistance for all members of the household that require food stamps or Medicaid.


3. Gather supporting documentation to be submitted directly to your local or state Medicaid office. You will need a state-, federally-, or military-issued photo identification card, Social Security card, birth certificate, proof of income and resources--and, if applying for Medicaid on the basis of disability, a listing of the names, addresses and phone numbers for all medical providers. You will receive a response to your food stamp application within 30 days and Medicaid within 90 days. If you are denied benefits, proceed to Step 4.


4. Write an appeal letter within 10 days of receiving notice of denial and submit this letter to your local Medicaid office or to the address listed on the letter itself. This appeal letter should state your full name, Social Security number and date of birth and provide a list of which programs you are appealing, as well as reasons for appealing the denial. Submit documentation supporting your appeal, such as additional medical records if denied on the basis of not being found to be disabled. You may have to attend a hearing with an administrative law judge as part of the appeal hearing process. During this hearing, you will be asked to provide a statement regarding your reasons for denial and will need to present a case stating the reasons why you feel that you should not have been denied benefits.
